Certain headings may not appear among the loaded authority records, and should not be added as new authority records. These consist of certain phrase headings that are composed and applied in bibliographic records as required. These headings are:
| [Name of city] | Metropolitan Area ([Geographic qualifier]) |
| [Name of city] | Region ([Geographic qualifier]) |
| [Name of city] | Suburban Area ([Geographic qualifier]) |
| [Name of geographic feature] | Region ([Geographic qualifier]) |
While some of these phrase headings may appear in the index, authority records do not need to be constructed for them. The portion of the heading which is displayed in square brackets in the above list should be checked. It should match the appropriate name heading. The same geographic qualifier used for the name heading is also used for the phrase heading. If no geographic qualifier is given to the name heading, no geographic qualifier is added to the phrase heading.

Several additional types of headings used to be treated as phrase headings, but are now established individually as required by the Library of Congress. These include:
| [Topic] | in art |
| [Topic] | in literature |
| [Name of river] | Delta |
| [Name of river] | Valley |
| [Name of river] | Watershed |

At various times, the following phrase headings were also constructed. These headings were placed in 650 tags, even though the initial portion of the heading was constructed from a personal, corporate or place name.
| [Personal name] | in fiction, drama, poetry, etc. |
| [Name] | in art |
| [Name] | in literature |
The free-floating subdivisions |x In art and |x In literature are now used under personal, corporate, etc. name headings.
